Hey Lang, No we replaced the thermostat in Toowoomba after the first overheating incident. A couple of people have asked about the circumstances well, here goes..
1st time we had been stuck in Toowoomba for a few days after the slave cylinder failed on the bypass, we left BDS, shot down to the 24hr United and by the time we got back to the top of Taylor street, it was spitting water out of the filler. BDS spent the whole next day trying to replicate with no luck, replaced the thermostat as a precaution. Approximately 6t load.
Next day we began our trip to Darwin, couple of minor temp spikes, but as soon as the thermostat opened it instantly dropped back down to 80 degrees.
Drove back to Darwin without fail.
Did a few small jobs around Darwin without fail, ran a container from Palmerston to Dundee, loaded a 11.5t crane on and delivered it to Darwin River, noticed the guard was wet, but temp hadn't changed. Swung into berry springs pub for lunch, half an hour, maybe more and then the second time was on the way home empty and somewhat cool.

You mentioned truck was modified by previous owner. Was the cooling system modified?
I added a header tank to cooling system on my 4B Cummins powered Manitou telehandler, (rather than standard radiator cap and recovery bottle which was very hard to access) and it immediately started blowing coolant out. I found the water pump was sucking air in through a small air-bleed hose that ran up to the header tank, (and vented to atmosphere).
And so i fitted a 1/4 turn tap in that hose, so i can still bleed air out when filling the system with coolant, but then seal that hose off during normal running, to prevent air being sucked in.
I am wondering if you have a header tank, whether coolant level is the issue? Perhaps below a certain level a similar air bleed hose is exposed and sucking air? Maybe you have trouble once, and top up coolant, then all is good again for the next while, until perhaps unknown to you, the level drops just enough to expose/open a similar air-bleed hose, and you have trouble again.
Just a random idea, for what seems a random problem.

Not a bad thought actually! I'll have to check into that. There's definitely a header tank that sits at roughly the same height as the filler, except the filler is lower, hence why it pumps out of there and not the header tank.

I would have expected the filler to be sealed with a flat cap (no spring) and the pressure cap to be on the coolant/overflow tank.
I run two engines with a pressurised coolant tank and one with a true overflow/catch tank. All 3 vehicle are modified.
None of them ever spits out coolant.
I think there's something amiss in the overflow/catch tank set up.

I agree with Zuffen, i would expect the "filler" you mention to be sealed, and the only fill point, and cap, to be on the header tank, that being the highest point of the system.

I'm not sure why the blue cap on your header tank isn't the "filler"?
I don't know if your current "filler" is standard, or a modification just for easier access?, but it just seems wrong to me.
I am going to stick with my "slightly too low" coolant level theory for now.
